- After your evaluation has concluded and you've been verbally approved, you'll need to submit payment. This is an automated process as you'll need to put in your credit/debit card details before starting the evaluation. Your debit/credit card will NOT be charged unless you are approved
- By email, you will receive a completed and signed version of the Missouri Physician Certification Form form which is needed to obtain your card to the email address you designated.
- You will submit this form to the Missouri DHSS portal which can be found here. Click the blue "PATIENT/CAREGIVER APPLICATION REGISTRATION" button and follow the registration instructions.
