The process is very streamlined and efficient.

  2. Complete the quick intake information regarding which covers your basic data and medical history
  3. Start your online telemed consultation with the physician. During your consult, the physician will review the information you submitted, ask questions about your medical history, gain an understanding of the ailments you suffer from and answer any questions you have about the medical marijuana program and Missouri
  4. After your evaluation has concluded and you’ve been verbally approved, you’ll need to submit payment. This is an automated process as you’ll need to put in your credit/debit card details before starting the evaluation. Your debit/credit card will NOT be charged unless you are approved
  5. By email, you will receive a completed and signed version of the Missouri Physician Certification Form form which is needed to obtain your card to the email address you designated.
  6. You will submit this form to the Missouri DHSS portal which can be found here. Click the blue “PATIENT/CAREGIVER APPLICATION REGISTRATION” button and follow the registration instructions.

The state requires a $25 application fee for all new patients, this fee is annually.

If you need to add a caregiver to your medical marijuana account, the application fee for this individual is also $25 per year.

Application fees must be paid by Visa, MasterCard, or Discover credit or debit card. An additional credit card processing fee will be added to the transaction. These application and processing fees are non-refundable. The payment will not be refunded for applications not approved.

No. Medical marijuana is still technically ‘illegal’ at a federal level, therefore insurance companies must comply with federal rules and regulations, not state.

Step 1: Forms

To begin, patients will need the Physician Certification Form completed by an MD or DO in good standing in the state of Missouri.

For patients looking for the standard recommended dose of four (4) ounces per month, use the Standard Physician Certification Form.

Patients requiring more than the standard four (4) ounces should use the Alternative Physician Certification Form, and are required to submit two of these forms, completed by two different physicians, with the same qualifying medical condition selected on both forms. If the physicians’ recommended amounts listed on the forms are inconsistent, the patient will be allowed the lesser of those two amounts.

Applying caregivers will need a Patient Authorization Form and the patient’s license number, which the patient will receive once their application is approved. This number will begin with PAT.

 

Step 2: Register

To complete an application, patients will need to register with the Complia application portal.

Click on the Patient/Caregiver Application Registration blue box located under the photograph to apply for your identification card. If a “Warning-Exiting Site” pop-up box appears, click “Yes” to proceed to the registry. Only applications submitted through the on-line registry will be accepted.

 

Step 3: Application

Once patients have registered, an application can be completed by logging into the Complia portal, clicking “+Create Application” and “New Patient Registration”.

    3. A clear, color photo of the qualifying patient’s face taken within the prior three (3) months.

    1. At the option of the applicant, a statement indicating whether the qualifying patient is currently receiving assistance from any Missouri programs for low-income individuals, and if so, which programs.
    2. If the patient is seeking authority to cultivate medical marijuana.
    3. Attestation statement; Signature and date of the application.
    4. All applicable fees.

Yes, upon receipt of a patient cultivation identification card. There is an additional $100 fee to grow up to six flowering plants per patient and security regulations that must be followed. See 19 CSR 30-95.030(4) for more information.

The qualifying patient’s physician may certify up to four ounces of dried, unprocessed marijuana, or its equivalent, in a 30-day period. If there is a compelling reason why the qualifying patient needs a greater amount, then the Department requires two independent physician certifications specifying what other amount the qualifying patient needs. Yes. Previous convictions do not disqualify an individual from obtaining an identification card. However, a qualifying patient’s identification may be revoked for certain criminal conduct that occurs after the card is issued. See 19 CSR 30-95.030(3)(B) for more information.

  • 2. Responsible for managing the well-being of a Qualified Patient; and
  • 3. Designated on the primary caregiver’s application for an identification card or in other written notification to the department.

Yes. Patients and caregivers with a valid department-issued identification card may legally possess medical marijuana now subject to the requirements and limitations in 19 CSR 30-95.030 and Article XIV of the Missouri Constitution

Article XIV of the Missouri Constitution, which authorizes medical use of marijuana in Missouri, does not reference or prohibit the possession or purchase of firearms. However, federal law still may prohibit the possession or purchase of a firearm by individuals who use marijuana, regardless of whether a state has legalized marijuana for medical use. The Department of Health and Senior Services does not regulate the possession or purchase of firearms and therefore cannot say how the federal prohibition will be enforced within Missouri. Specific questions about these federal firearm restrictions should be directed to your attorney or the appropriate law enforcement agency.
